I think the man on the left has a scrotum that is actually too large, looks like he is suffering from a medical condition, possibly a hernia but more likely an "edema" swelling from too much fluid accumulation in his scrotum. If he had a hernia this distended he would be in serious pain just standing up. I remember reading about the scrotal edema condition, it is often without any pain or discomfort, most men that have that condition just live with it but a surgeon can drain off the excess fluid and I really believe just for aesthetic reasons that should happen. The scrotum should "hang loose" and the skin conform to the surface of the testicles, this poor fellow looks more like a canvas water bag that is overfull.

another possibility is something I heard of (and have seen photos of) where a guy injects saline or distilled water into his own scrotum. It is a Kink practice. I have also heard of that condition, I think it is called a varicocele. I knew a man who had one and just lived with it. His urologist said that it could be drained, but felt that the risk of infection was too great. He never had any pain or discomfort.
I think I would have to invest in a larger and more padded bicycle seat before attempting a bike ride with balls that big. The World Naked Bike Ride in Philadelphia a few years ago convinced me that 13-mile rides can be a little rough, even for smaller dicks.
I know that I am alone in this opinion, but it is just me so here goes; ever since I was about three years old, I have really disliked the look of my own genitals, no problem with others, just my own. I always felt that my scrotum hung down way way too low and just looked grotesque. If I could afford the cosmetic procedure, I definitely would get what is called "scrotal reduction surgery" and it is usually done for older men who want to appear younger. When it is warm, my balls hang a bit. Using testicle stretchers has probably added to my scrotum length. Have you tried cock rings? They tend to decrease the scrotum's hanging appearance and in my opinion also feels good. Go with the stretchy ones.
