It might be the one behind him biting his neck.
BTW, the person behind him, which I doubt is biting his neck, is male. If you look you can see a bit of a beard.
Probably he lost a bar game to them.
The girl on the right just seems unimpressed by what she sees. She was hoping for thicker.
