I think the weights are attached to the skin of the scrotum with clamps. They don't hurt and don't touch the balls, but they certainly make everything visible and a bit humiliating.

But from my experience attaching weights with clamps on the skin of the scrotum can work but not very well and the average man would not be able to do it for long. The artist probably did not have actual experience with this and they took "creative license".
There are many ways to attach weights to balls and I have explored several of them. What works best for me, is to encircle both balls with a cord or with a metal ring of some sort which is preferred. It pulls down on the whole scrotum and the balls are still mostly visible.
